1. Please send hard copy of this form to the Organizing Secretary.

2. Last date of registration is 10th of December 2016. After this date, registration will be done on the spot, i.e. 27th of December 2016 and there will be no commitment to accommodation.

3. Delegates shall have to send their fee to the following Bank account either through internet banking or through Bank Deposit Fee. If you are depositing cash in the bank please note that banks charge service tax and transaction charges. Please inquire from the bank what is its service and transaction charge and add these to the amounts to be transferred before depositing the cash. For example, SBI charges Rs.57.50. Thus if you are a Life Member and registering with accommodation before 30 September, you will have to deposit Rs. 2557.50/

4. Entry of the name of the delegate is mandatory in the ‘narration line’ while making payment either online or in the bank.

5. Please note that No cash or draft will be accepted for Registration

6. Last date for confirmation for the print copy of abstracts and transport arrangement is 20th of November 2016.
